NO SHOT, NO SCHOOL
Please turn in shot records as soon as possible.
During the summer, when school offices are closed, shot records may be turned in at the District Offices: 921 Susana Street: Attention Health Services.
For the 2012-2013 school year and for all future school years, all incoming 7th grade students will need proof of a whooping cough booster shot before starting school. Whooping cough, also known as Pertussis is a bacterial infection that can easily spread causing severe coughing fits and missed days of school. The cough can last for several weeks. Whooping cough is a widespread illness in California. The booster shot must have been given on or after the 7th birthday and verification of that shot is to be turned into the school. Adults may also receive the vaccine for the benefit of their health and protection of others in the family.
You may also fax or email the shot record to the District Nurse. Fax: 925-313-0753 or email to: schoolshots@martinez.k12.ca.us. Students who have not received this booster shot by the first day of school, will not be allowed to start classes. There is no grace period for this requirement. Proof of the immunization must be turned in to the school's counseling office as soon as possible.
We encourage you to contact your child’s doctor now to schedule an immunization appointment or to ask if your child has had the shot. Children who received a dose of Tdap on or after their 7th birthday meet the requirement.

TheCoordinated School Health Program evolved in response to changes in status ofchildrens health and education. A coordinated school health program utilizes personnel, agencies, and programs, both in and out of the school building that relate to student health and success in school.
